About suffixes:

If you have had trouble finding your voter registration and your name has a suffix, such as "Jr." or "III," include this suffix after your last name. For example, if you are John Smith, Jr., enter "SMITH JR" as your last name.

If your suffix has only one digit, such as "I" or "V," do NOT include the suffix in your search.

To help us confirm your status, please list your driver's license number (if you have one) and the last four digits of your Social Security number. If you have a state ID, you may list that number as well.
